A design failure modes and effects analysis (DFMEA) is a top-down inductive analysis technique well suited to looking at the response of a system/sub-system/component to a failure input. Pi Innovo starts the DFMEA process by doing a careful analysis of the boundary of the system under analysis. This is described in diagram form within the DFMEA template.
Once the boundary is determined, techniques such as a P-diagram are used to identify all the input signals, output signals, noise factors, control factors, error states, and system responses. These steps help to identify the full set of parameters that the DFMEA will then use in the analysis.
The DFMEA is a team activity. Since the system under analysis has multiple disciplines, such as electrical and mechanical, so does the DFMEA team. The DFMEA team may also require customer participation either while conducting the analysis or to perform a review.
